Enrollment in the Marketplace

Open enrollment begins November 1, 2016 for coverage beginning as early as January 1, 2017. Open enrollment through the Marketplace ends January 31, 2017.

• When you apply for coverage in the Health Insurance Marketplace, you’ll need to provide some information about you and your household for example, total household income and any insurance you may currently have.
• Social Security Numbers (or document numbers for legal immigrants)
• A contact telephone number for the Employer for every job-based plan you or someone in your household is eligible for. You’ll need to fill out this form even for coverage you’re eligible for but don’t enroll in and employer’s phone number.
